Chapter 1: What Just Happened to Your Heart

Your heart did something unexpected, and now you have a name for it. That name can feel like a verdict. This chapter is about making sure you understand what the diagnosis actually means, what it does not mean, and why the difference matters for everything that comes next.

A disorder of electricity, not of plumbing

Most people, when they hear that something went wrong with the heart, picture a blocked artery or a heart attack. Atrial fibrillation is neither. The arteries supplying your heart may be perfectly clear. The muscle itself may be pumping with normal force. What has gone wrong is electrical: the signals that tell your upper heart chambers when and how to contract have become chaotic rather than organized.

That distinction matters because it shapes the treatment, the outlook, and the way doctors will talk to you about risk. AFib is serious and worth taking seriously. But the path forward looks quite different from the path after a heart attack, and knowing that can change how you receive the information in this book.

How a healthy heartbeat is made

Every beat of a healthy heart begins in a small cluster of cells high in the right upper chamber. This cluster is called the sinus node, and it functions as the heart's natural pacemaker, generating a steady electrical pulse roughly 60 to 100 times a minute at rest. That single pulse spreads outward in an orderly wave, causing both upper chambers, the right and left atria, to contract at the same moment and push blood down into the lower chambers, the ventricles. The electrical signal then funnels through a relay point between the upper and lower chambers called the atrioventricular node, or AV node. The AV node slows the signal slightly, giving the ventricles time to fill before they contract and send blood to the lungs and body. The whole sequence, from sinus node pulse to full ventricular beat, takes less than a second.

When that sequence works correctly, the rhythm is called normal sinus rhythm. Every chamber does its job in the right order. Blood moves through cleanly.

What goes wrong in AFib

In AFib, the sinus node does not stop working, but it gets overwhelmed. The trouble usually begins near the pulmonary veins, the four vessels that carry oxygen-rich blood from the lungs into the left atrium. Tissue just where those veins meet the atrium can fire rogue electrical signals, not one organized pulse but dozens or hundreds of chaotic ones per minute. Those signals race across both atria simultaneously, from multiple directions at once, faster than any organized contraction can follow. The atria stop squeezing and start quivering, a fine trembling motion that moves very little blood and moves it inefficiently.

The AV node, which normally acts as a gatekeeper, receives all of those chaotic signals at once. It cannot process them all cleanly, so it passes some to the ventricles in a fast, irregular pattern. The ventricles respond: they beat faster than normal and without a predictable rhythm. Most people describe this sensation as a racing, fluttering, or pounding in the chest, a feeling that the heart is skipping or stumbling. That sensation is AFib.

How common this actually is

AFib affects an estimated 1 in 22 American adults, roughly 10.55 million people, according to a 2024 National Heart, Lung, and Blood Institute report. That makes it the most common serious heart rhythm problem in adults. The number matters less as a statistic and more as a reminder: the path from diagnosis to effective management is well-traveled. Cardiologists and electrophysiologists treat this condition every day. The tools available now, the medicines, the procedures, the monitoring devices, are more precise than they were even a decade ago.

The symptom gap

Not everyone with AFib feels it. Some people find out during a routine physical when a doctor notices an irregular pulse, or during an EKG taken for an unrelated reason. Others feel palpitations so clearly they could time them. Still others notice fatigue, a vague heaviness during exertion, or shortness of breath that they had been chalking up to age or fitness.

The absence of obvious symptoms does not mean the condition is mild or that it can be ignored. Blood can pool and clot in the upper chambers whether or not a person feels the irregular rhythm. The risk that comes with AFib, primarily the risk of stroke, exists regardless of how the rhythm feels from the inside. Chapter 5 covers that risk in full, because understanding it is central to understanding why treatment matters even when symptoms are minimal.

One more thing about your heart rate during AFib

Doctors often aim to bring the ventricular rate within a general target range during AFib. Published guidance typically describes a resting target of roughly 60 to 100 beats per minute for most people managed with rate control, though your clinician will set a specific goal based on your individual situation. One important caveat worth knowing now: if you are taking a beta-blocker (a common medicine in AFib care), your pulse may read within that range even during a symptomatic episode. If you feel something that worries you, trust that feeling and call your care team; do not rely on your pulse number alone to tell you everything is fine. Three goals that organize everything

Once AFib is diagnosed, treatment organizes around three questions:

How fast is the heart beating? The chaotic signals pushed through the AV node can make the ventricles race. The first priority is often slowing that rate to a range that lets the heart fill properly and prevents the sustained fast rate from weakening the heart muscle over time.

Can the normal rhythm be restored? For many people, doctors will discuss whether it is possible and worthwhile to bring the heart back to a steady sinus rhythm. This is called rhythm control. The right answer depends on the type of AFib, how long it has been present, the person's symptoms, and other health factors. The 2023 ACC/AHA/ACCP/HRS guidelines marked a shift toward considering rhythm control earlier in the course of the disease rather than only after multiple medication attempts have failed.

How can stroke risk be reduced? Because blood pools in the upper chambers during AFib, clots can form, and those clots can travel to the brain. Managing that risk, usually with anticoagulant medicines, is a central pillar of care for most people with AFib. Your doctor will assess your individual risk level using a published scoring tool called CHA2DS2-VASc, which weighs factors such as age, sex, blood pressure history, and prior stroke, then discuss whether the benefit of an anticoagulant outweighs the bleeding risk for you specifically.

These three goals, rate, rhythm, and clot prevention, appear throughout this book. Each gets its own full treatment in Chapter 8. The reason they are introduced here is that every conversation you have with your doctor will circle back to at least one of them. Knowing the framework helps you follow those conversations and ask better questions.
